Tiivistelmä
Työssä käsiteltiin diagnostiikkalaboratorioon aiemmin hankitun erillisen automaattisen käsittelijän käytön laajentamista verenkuvanäytteiden esikäsittelyyn ja tämän vaikutusta verenkuvanäytteiden läpimenoaikaan. Viitekehys koostui lean-ajattelun jatkuvan parantamisen menetelmien ja esikäsittelyprosessin yhdistämisestä. Tutkimus oli yhdistelmä tapaus- ja toimintatutkimusta PDCA-mallia mukaillen. Mukautettua arvovirtakuvausta käytettiin prosessin eri vaiheiden kuvaamiseksi ja analysoimiseksi.
Työssä havaittiin, että automaattisen esikäsittelijän käyttö verenkuvanäytteiden esikäsittelyyn pidensi näytteen läpimenoaikaa, mutta henkilöstön haastattelun ja kirjallisuuden mukaan potentiaalisesti vähentää virheitä prosessissa. Esikäsittelijän käyttöä ilman analysaattoreihin liitettyä rataratkaisua ei kuitenkaan voida pitää lean-ajattelun mukaisena arvoa tuottavan aikana, koska näyte ei vaadi sentrifugointia, eikä näytteen läpimenoaika lyhene. Nykyisessä prosessissa hukkaa syntyy paitsi näytteen kuljettamisesta, niin myös odottelusta ja ylimääräisestä liikkeestä, näytteen siirroista laboratorion sisällä.
Lean-ajattelun mukaisesti seuraava vaihe läpimenoajan lyhentämiseksi olisi joko rataratkaisu esikäsittelijän ja verenkuva-analysaattorin välillä tai laboratoriotilojen uudelleen järjestely hukan poistamiseksi. Ratkaisuksi on valittu laboratoriotilojen remontti, jonka seurauksena esikäsittelijä ja verenkuva-analysaattori saadaan lähemmäksi toisiaan. Niiden välinen etäisyys lyhenee remontin myötä 40 askeleesta noin 15 askeleeseen (63 %). Työssä laadittiin arvovirtakuvaus tilanteesta ennen ja jälkeen esikäsittelijän käytöstä verenkuvanäytteiden käsittelyyn sekä tulevaisuuden tavoitetilasta. Tavoitetilassa läpimenoaika lyhenee arvion mukaan 1–35 minuuttia (8.3 %) nykyisestä. Yhden minuutin aikasäästö arkisin klo 7-15 analysoitavien putkien osalta olisi karkeasti arvioiden maksimissaan noin 27 minuuttia päivässä. This thesis examined expanding the use of a previously acquired stand-alone automated processor in a diagnostic laboratory to include the pre-analytical processing of complete blood count (CBC) samples, and the impact of this on sample turnaround time. The the-oretical framework consisted of combining Lean continuous improvement methods with the pre-analytical process. The study used a combination of case study and action rese-arch methodologies following the PDCA cycle. A customized value stream map was used to describe and analyze the different stages of the process
The study found that using the automated processor for CBC sample pre-treatment lengthened the sample turnaround time, but according to staff interviews and literature, it potentially reduces process errors. Utilizing the pre-analytical processor without a track system connected to the analyzers cannot be considered value-adding time under Lean principles, as the sample does not require centrifugation and the turnaround time does not decrease. In the current process, waste is generated not only by sample transport but also by waiting, unnecessary movement, and sample transfers within the laboratory.
In accordance with Lean thinking, the next step to shorten the turnaround time would be either a track solution or rearranging the laboratory layout to eliminate waste. The sub-sequent phase involves renovating the laboratory facilities, which will bring the processor and the CBC analyzer closer together. The renovation will reduce the distance between them from 40 steps to 15 steps (63 %). The thesis includes a value stream map of the si-tuation before and after implementing the processor for CBC samples, as well as a future target state. In the target state, the turnaround time is estimated to decrease by 1 to 35 minutes (8.3%) from the current level. A one-minute time saving for tubes analyzed on weekdays between 7 AM and 3 PM would be roughly up to 27 minutes per day
